Case Management

Assisting young people with goals in education, employment, housing, and more.

One goal of case management is to help young people transition into adulthood with confidence, self-reliance, and the knowledge of how and when to seek support.

Case managers build one-on-one relationships with young people and empower them to make informed decisions about their future. Youth and young adults are encouraged to explore options, learn problem-solving skills, and develop the confidence needed to take meaningful steps toward life milestones. We offer guidance while honoring each young person’s voice, goals, and pace.

What We Help With:

While our assistance covers many areas, here’s an outline of some core focuses:

Goals May Include:

  • Stable Housing – Always a top priority.

  • Employment – Preparation and training.

  • Educational Support – May include completing a high school diploma or GED, and/or pursuing college, vocational programs, or trade school.

  • Life Skills Development – Budgeting, hygiene, cleaning, managing household responsibilities, paying bills on time, and more.

  • Access to Vital Documents – Including other essential resources.
  • Family Reunification – When appropriate.
