I understand that the scope of the consumer report/investigative consumer report may include, but is not limited to the following areas: Verification of social security number Employment history Education background Character references Drug testing Civil and criminal history records from any criminal justice agency in any or all federal, state, or county jurisdictions, driving records, and any other public records. I further authorize any individual, company, firm, corporation, or public agency to divulge any information, verbal or written, about me, to Rod’s House or its agents. I further authorize the complete release of any records or data pertaining to me that the individual, company, firm, corporation, or public agency may have, to include information or data received from other sources. Rod’s House and its designated agents and representatives shall maintain all information received from this authorization confidentially to protect the applicant’s personal information, including but not limited to, addresses, social security numbers, and dates of birth. Our young people come from many different families, cultures, and communities. We celebrate diversity and nurture cultural and ethnic pride in the people we serve. Please listen and learn about the backgrounds and values of young people you interact with. Do not impose your personal values and beliefs onto the young people. Positive relationships allow young people to learn and thrive. Help staff create affirming environments by knowing and caring for the young people and sites you volunteer with. Choose a commitment level you can keep. We count on volunteers to be dependable and regular in attendance. Sign in and sign out; wear your issued Rod's House volunteer badge at all times. Volunteers are important role models for our young people. Familiarize yourself with the Rod's House rules and our values and expectations. Engage with young people in a respectful and age appropriate manner and maintain the highest professional, moral and ethical standards in your interaction with young people. Know and observe all regulations and procedures at all Rod's House sites. Do not take or post photos or share descriptions of young people on personal websites or social media accounts. Do not engage in commercial, political, or religious solicitation of young people or staff. Do not bring minors, including own children, when volunteering at Rod's House sites. Do not share your contact information with young people, including social media information. Discuss problems that arise with your point staff member. Report suspected abuse or neglect immediately to your point staff member or another Rod's House employee. We are mandatory reporters. Staff will do reporting and follow-up. Agreement to Volunteer Responsibilities *I have read and agree to the Terms of Service Agreement listed above.I agree to follow the above volunteer program participation requirements and remaining guidelines included in the volunteer handbook during and after my time as a volunteer for Rod's House. I acknowledge that I must complete the online training courses AND meet criteria for background check clearance prior to volunteering with Rod's House. This includes claims for property damage, personal injury, or wrongful death that may arise from my participation in Volunteer Activities, regardless of when, where, or how such claims may occur—even if caused by ordinary negligence. Assumption of Risk I understand that participating in Volunteer Activities carries certain risks, including the potential for serious injury or death. Indemnification I agree to indemnify and hold harmless Rod’s House from any claims, demands, or expenses, including legal fees, arising from my participation in Volunteer Activities. Insurance Acknowledgment I understand and acknowledge that Rod’s House does not provide health, accident, or liability insurance for me or the volunteer (if under 18), and I am solely responsible for securing any insurance coverage I may need. Severability This agreement is intended to be as broad and inclusive as permitted under applicable state law. If any part of this waiver is found to be legally invalid, the rest shall remain in full force and effect.
